Retailer Information:

  • Full name

  • Full address

  • Email

Required Details:

  • FIBERWOOD product category

  • Date of purchase, installation, and appearance of the defect

Documents to Attach:

  • Invoices from the official FIBERWOOD retailer (contractor invoices are not accepted)

  • Wide-angle photos of the affected area

  • 10 or more close-up photos clearly showing the observed defects
